SAFETY QUIZ

What’s Your
Safety Score?

The first step to improving your safety program is knowing where you stand.
Take our 5-minute quiz to see how effective your safety program is today and where it can improve.

Know where your safety program stands—and what to improve next

A safety maturity model scores how well your program actually performs—not how much activity it generates—on a defined 0–4 scale.
That turns “Where do we stand?” into a number with a next step instead of a feeling.
The score tells you where you are; the stage tells you what it means.

“Safety maturity shouldn’t be an opinion. It should be a score.” — Rob Cowden

The four Safety Plus maturity stages

0-1

REACTIVE

0 — Nonexistent: reactive, with no structure.
1 — Basic: checkbox safety—activity without management.
Next move: add structure through sequence, ownership, and records.

2

ORGANIZED

Developing: the transition from “we do safety” to “we manage safety.”
Next move: improve consistency and measurement.

3

SUPPORTED

Proactive: predictable and leader-driven, with leading indicators in balance.
Next move: integrate safety into business decisions.

4

COMPETITIVE

World-Class: safety is embedded in how the business competes.

Estimate where you stand

  • Could you produce your evidence package today without a scramble?
  • Do corrective actions close on schedule without heroics?
  • Do leading indicators get reviewed, or only incident counts?
  • Does the program run when the key person is on vacation?

Most small and mid-sized businesses honestly assess between 0 and 2. That is useful because each level has a known next move.

What the Safety Maturity Score measures

  • Leadership commitment
  • Hazard identification and risk control
  • Compliance and documentation
  • Training and engagement
  • Incident and near-miss reporting
  • Metrics
  • Program structure
  • Continuous improvement
  • Culture

How the overall score is calculated

Add the nine category scores and divide by 9. Example: 21 ÷ 9 = 2.3 when rounded to one decimal.

Audit score vs. maturity score

Audits check compliance at a point in time. Maturity scores rate the system’s capability—whether performance is designed in or depends on heroics.

The Safety Maturity Score measures your current program state. Reassess it as your program advances, so changes in the score reflect changes in how safety is managed.
